This page contains downloadable government electronic forms. Over the years, we have had requests for many of these, but they have never been available to depository libraries. This list is not comprehensive, but comprises those forms most asked for by the public. Many other agencies have quite specialized forms and should be accessed directly.

federal employment forms, including the OF-615. Please check
with your agency before submitting an SF-171.
Passport
Application Forms
Contains forms for new passports, as well as
renewals. Several other forms are available, too.

Trademark Office
This site contains an extensive listing of patent
and trademark forms, as well as a few Patent Treaty Cooperation Treaty
(PCT) Forms. A more complete listing of PCT forms, including those in
